Pre-Visit Health History Questionnaire

Completed digitally prior to first appointment. Captures chief complaint, weight history, prior weight loss attempts, medications, allergies, family history, and relevant comorbidities (HTN, DM2, PCOS, thyroid disorders, cardiovascular disease).

STEP 2

Biometric & Physical Assessment

Conducted at initial in-person or telehealth visit with self-reported/verified measurements. Includes all biometric data, vital signs, body composition, and anthropometric measurements.

STEP 3

Laboratory & Diagnostic Work-Up

Comprehensive metabolic and hormonal panel to establish baseline health markers and identify contraindications or conditions that may influence therapy selection.

STEP 4

Clinical Risk Stratification

Provider assigns patient to Low, Moderate, or High risk category based on BMI, comorbidities, labs, cardiovascular status, and history. Determines appropriate therapy tier, monitoring intensity, and follow-up frequency.

STEP 5

Individualized Treatment Plan & Informed Consent

Personalized protocol selected and documented. Patient receives full informed consent covering mechanism of action, expected outcomes, dosing schedule, side effects, monitoring requirements, and distinction between FDA-approved and non-FDA-approved agents.

STEP 6

Ongoing Monitoring & Reassessment
